You can click "Title 1" to start changing the form's title. For example, we use the title "HIPAA Authorization Form."

You can then drag the Input Field from the left panel to the right panel preview to add a text box.

Then, scroll through the customization panel and click "Save Changes" to save your settings.

Next, you can add disclosures of protected health information. You can add multiple Display Options to create multiple-select fields.

You can also add more options by clicking the "+ Add Option" button. Then, simply fill in the details required for your selection. When you're ready, don't forget to scroll down and click "Save Changes."
